Description
The Visit Nurse (RN) is responsible for providing nursing services to Hudson Valley Hospice patients and their families and to ensure that those services are consistent with the philosophy, goals, objectives and policies of the agency. Work closely with the interdisciplinary team, under direction of RN leaders, to ensure patient needs are met. This position will travel throughout Dutchess County, and visit patients wherever the need is.
What you will do:
- Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural, and spiritual assessments to inform development and revision of the individualized plan of care based on the needs of the patient per assigned visits.
- Ensure and coordinate the implementation and delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team
- Provide palliative nursing care and symptom control in accordance with physician orders and plan of care.
- Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur.
- Teach, supervise, and counsel patients/families/caregivers regarding physical care, observing return demonstration of procedures as appropriate.
- Effectively communicates patient status to family members and/or caregivers.
- Maintain adequate records of all patient/family care services, as defined by policy.
- Collaborates with interdisciplinary team members to meet patient needs.
- Provides 24-hour continuous skilled nursing care for patients/families in their homes, as needed.
- Supports patient visit needs including but not limited to admissions, routine visits, symptom management visits, on call visits, and death visits; may support in managing patient caseloads and responding to patient/family needs in accordance with the POC.
- Maintains a working knowledge re: use of CADD-Prizm pump.
- Follows and upholds all state and federal laws and regulations, and HVH policies and procedures
Skills we are looking for:
- Ability to communicate effectively verbally (in person or by phone) and in writing.
- Capability to work effectively and cooperatively independently and as a team member.
- Expertise in prioritizing and handling multiple tasks efficiently.
- Capability in caring for terminally ill patients and a basic understanding and commitment to the hospice philosophy.
- Working understanding in the effective use of medications, treatments, and therapies for pain and symptom control.
Work Environment:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
- Moderate degree of possible exposure to highly contaminated agents.
- Regular travel by car in all weather conditions.
- Work in all locations of care including patient homes, hospitals, and nursing homes.
- May travel in both service counties.
- Moderate to high risk of exposure to unsafe environments.
- Routine duties involve the potential for direct exposure to blood or body fluids.
- Ability to carry up to twenty-five pounds of equipment and/or supplies into a patient’s home and transfer/position an average adult for care.
Preferred Education & Experience:
- Graduate from an accredited nursing program
- Valid NYS Nursing License
- Minimum of two (2) years experience in nursing
- Experience in oncology, home care or a hospice setting preferred.
- Experience working in a comprehensive interdisciplinary team approach to care preferred
Hours: Monday-Friday (8:30am-4:30pm); on call rotation (1-3 times per year on average)
FLSA: Exempt
Pay: $82,000-90,000 per year
