Remote (Must be authorized to work in the U.S.; preference for candidates familiar with Massachusetts healthcare environment)

Contract (Part-Time / Full-Time Options)
Compensation:
$18 – $25 per hour (Based on experience & certifications)
About the Role:
We are seeking a detail-oriented and compassionate Remote Care Coordinator to support patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.
You will work closely with patients, providers, and care teams to ensure continuous monitoring, engagement, and compliance with CMS guidelines while improving patient outcomes.
Key Responsibilities:
CCM (Chronic Care Management):
• Conduct monthly non-face-to-face care coordination (20+ minutes per patient)
• Develop and maintain comprehensive care plans
• Perform medication reconciliation and adherence tracking
• Coordinate with PCPs, specialists, and caregivers
• Document all interactions in EMR in compliance with CMS requirements
• Identify abnormal readings and escalate to clinical staff
• Educate patients on device usage and adherence
• Ensure 16+ days/month data compliance for RPM billing
• Maintain timely documentation for CPT compliance (99453, 99454, 99457, 99458)
Qualifications:
Required:
• Experience in CCM and/or RPM programs
• Strong understanding of CMS billing and documentation requirements
• Familiarity with EMR/EHR systems
• Excellent communication and patient engagement skills
• Ability to manage multiple patients and meet time-based care requirements
Preferred:
• Certification:
CCM (Certified Case Manager) or equivalent
• Clinical background:
RN, LPN, Medical Assistant, or Care Coordinator
• Experience working with Medicare populations
• Knowledge of Massachusetts healthcare workflows (preferred but not mandatory)

• Remote work with secure HIPAA-compliant setup
• Flexible scheduling, but must ensure monthly patient contact compliance
